Chronic conditions can last for some time and often need to be managed throughout life. These one-hour, online classes provide the tools, resources and support you need to help you feel your best.

Diabetes

Left unmanaged, diabetes can lead to problems like blindness and kidney damage. Take proper steps now to lower your risk or better manage diabetes.

COPD

Chronic obstructive pulmonary disease refers to a group of diseases that cause airflow blockage and breathing problems. Although there is no cure for COPD, it can be treated and managed.

Congestive Heart Failure

With congestive heart failure your heart muscle doesn't pump blood as well as it should. Typically, this occurs as a result of a heart attack, but it can also happen because of ongoing high blood pressure.

Weight Management

Keeping a healthy weight is important for overall health. Being overweight or obese puts you at risk for many diseases. The best way to lose weight is through education combined with consistent diet and exercise.

Kidney Health

The role of the kidneys is to remove waste products and extra water from the blood. Chronic kidney disease occurs when the kidneys don't work as they should and waste products start to build up in the blood.

Heart Health

High blood pressure (or hypertension) and high cholesterol can increase your risk of heart disease. If not treated, this can put you at a higher risk for heart attack and stroke. Lifestyle changes and medicine can help.
